Whether you prefer the privacy of your own home to one of our clinics, are looking to save time, or struggling with lack of mobility or transport, a home visit can be beneficial in multiple ways. Our team members will bring any equipment needed for your training sessions such as weights, bands, steps and more, and follow strict COVID-related protocols.

All home visits are 45 minutes in duration, and available from Tweed Heads in northern NSW to Beenleigh on the northern side of the Gold Coast.

Home-based rehabilitation from illness or injury can be a great way to recover in the comfort of one’s own home by having access to experts in familiar surroundings. Rehabilitation at home also allows for more freedom; clients are able to choose their own schedule according to what works best for them. It also enables family members or caregivers to get involved with treatment, providing moral support and helping our exercise physiologists to create a plan that is customised to every individual.

WHAT DOES OUR Home Care Offer?

Home visit packages vary depending on the allied health profession involved. Currently we offer exercise physiology at home but soon we will be offering home visits for physiotherapy as well. Our home visits take into consideration an individual’s needs and goals and involve the following steps:

Initial Assessment: Our allied health practitioners will always begin your health journey by conducting an initial assessment. We will start by looking into the client’s medical history, current health status, relevant lifestyle factors, and conduct key tests to set a base before discussing goals and pathways. Sometimes, we call this initial assessment a family consultation, because it can include not just the client, but family members or carers as well, providing the opportunity for questions be asked so that everyone becomes familiar and comfortable with the planned approach to the health and wellness journey.

Goal Setting: Based on this assessment, we will work with the client and family to set specific, measurable, achievable, relevant and time-bound goals.

Treatment Planning: Our practitioner will develop a treatment plan that is tailored to the individual’s specific needs and goals. They will also provide the necessary education needed to progress on this plan. 

Monitoring Progress: We will schedule follow-up sessions to monitor progress, adjust the treatment plan as needed, and provide ongoing support and guidance.

  • Our home plans are tailored to an individual’s needs, goals, and abilities and take into consideration any medical condition, physical limitation as well as personal preferences and abilities.
  • Depending on the nature of home care, the exercise plan includes a variety of exercises including cardiovascular exercise, strength training, and flexibility exercises.
